It's called: AquaPlanner! Comes in both a lite version for Nano Tanks (.99) or the pro version for larger tanks ($2.99)
This is a sweet lil app that I personally am having a blast with :)

I will second that. Very cool little app and worth the purchase. I love setting up reminders to help me stay on track.
