How do you all combat it? Do you frag it well above the disease tissue? Dip it? Frag then dip it, post your success/ and what didnt work so well.

150g 1" acrylic Sps reef mp40 and 4 k4s for flow. Aquaeuro 400 skimmer and 135 calcium reactor. 2 250w radiums on galaxy ballasts.
Reason I asked is i am having a couple of my well established pieces(2+ years ORA tri color validia and ORA Miami orchid ) showing STN with no pests in tank. I have dipped them several times and know the signs to look for with no success in stopping said event.
Parameters are

Sorry steady necrosis. I have had great water stability over the last few months when i was having this issue, i am hoping it was due to incorrect spectrum do to shifting of the bulbs. I just replaced them 2 weeks ago and waiting to see if that fixes the issue.
